2003 AGM REGISTRATION FORM

The USQRA will hold its Annual General Meeting at 9:00 AM on Thursday, April 10, 2003 at the Phoenix Airport Marriott Hotel. Each team in the USQRA is encouraged to send a representative to participate and vote on important issues that pertain to your sport and association.

If your team is sending a representative to the AGM please complete this form and return it to the USQRA by Wednesday, March 26th. Each team is allowed two representatives at the meeting, however, only one may vote on that teams behalf.

Available USQRA Tournament Package

If your team is not playing at Nationals you will only receive airport transfers to/from the hotel and lunch at the AGM. A complete USQRA tournament package is available from AZPVA for only $65 per person, and includes the following:

Round trip airport transfers to and from the host hotel
Daily shuttle service to the playing venue all weekend long.
One ticket to the USQRA Awards Dinner to be held on Saturday evening.
Free daily meals served at the playing venue during Nationals.
Free tournament T-shirt and official program.
