2003 USQRA / BlazeSports Club Clinic
Grant Request Form

Overview
The U.S. Disabled Athletes Fund, Inc.; BlazeSports of America; and United States Quad Rugby Association have budgeted for numerous clinics to be held throughout the United States.   These clinics will help provide instruction, coaching, and organized wheelchair rugby experiences to new and developing athletes as well as those desiring to reach an elite level. The USQRA / BlazeSports of America national clinic program is designed to provide funding for specialized sport clinics (3 days in length) in various areas throughout the nation. 

The clinic grants are available to four BlazeSports Clubs of America programs and one to two non affiliated sites.  The grant amounts will range from $1,000-$2,000 each.  BlazeSports of America encourages the utilization of its clinic grant funding as a source of matching funds to be generated from other funding sources in the local community. Clinic grant applications will be reviewed and ranked on a competitive basis against proposals submitted by other BlazeSports Clubs and non affiliated organizations. The applications will be reviewed and awarded by the USQRA Board and a BlazeSports Clubs of America representatives.  

Clinic grant funding can be utilized to support the clinic expenses such as coaching subsidies, coaching staff travel and lodging, training materials, clinic equipment and supplies, and other expenses directly related to the success of the BlazeSports Specialty Clinic. 

Grant Limitations
The U.S. Disabled Athletes Fund and BlazeSports Clubs of America awards grants to 501(c)(3) tax-exempt organizations eligible for funding under the United States Internal Revenue Code or other bona-fide local government agencies. 

Grant Application Deadline
Application must be postmarked by May 31, 2003
 

Application Package/Process
Enclose Two copies of the following items (incomplete proposals will not be reviewed):

1.      Two copies of a completed Clinic Grant Application, available in Microsoft Word on the www.quadrugby.com or www.blazesports.com websites.

2.      An introductory letter describing the purpose, amount of the request, and any additional information on the event not included on the application.

3.      The organization's 501(c)(3) statement/letter from the Internal Revenue Service or in the case of a government agency a statement in the introductory letter indicating the government jurisdiction in which you are a department.

4.      A Project Budget.

5.      Please list potential funding sources and whether or not matching funding is committed, pending or applied for.

6.      A simple evaluation form. What outcomes does the organization hope to achieve with the help of this grant?

7.      If the organization will be working in schools or with other nonprofits in partnership, please include a list of partners, along with contact information.

8.      If the organization has received a grant in the past, please include a line item account of how the first grant was used.

9.      Each organization awarded a grant will be required to sign a letter of agreement with USQRA and USDAF.  Additionally, the funded organization’s Club will be required to submit an event report and final budget reflecting how the funds were spent.
