2008 USQRA NATIONALS - Louisville, Kentucky Frazier Rehab Institute is proud to be the host for the 2008 USQRA Division I & Division II National Championships on April 11–13, 2008. We would like to congratulate all the teams in qualifying for the National Tournament and welcome any new teams attending as well. We look forward to seeing everyone in Louisville, Kentucky-home to the 134th Kentucky Derby. As with any National Tournament there are a lot of details. We have tried to list below most of the important information regarding the 2008 National Tournament to assist you in your preparations to attend. Additional information will be faxed or e-mailed to all teams as they confirm attendance at the 2008 National Tournament. If you have questions about the 2008 National Tournament which are not answered here please don’t hesitate to call us at (502) 582-7618 or (502) 582-7411. LODGING: We are pleased to announce that the Host Hotel for the Tournament will be the Executive West Hotel (www.executivewest.com) located at 830 Phillips Lane, Louisville, Kentucky 40209. A group rate of $85.00 (plus a 15.01% Hotel Tax) per night for a standard room has been secured for this event. A variety of one and two bedroom suite options are available at additional prices ranging from $105.00 to $240.00 (plus a 15.01% Hotel Tax) per night depending on the option you choose. To reserve your rooms please call 1-800-626-2708 or (502) 367-2251 by Friday April 1st and request a reservation for the “USQRA Nationals”. If you wish to make your reservation on-line please go to the following web link:www.exwesthotel.com. You must use a valid credit card and the Group ID# ‘487558’ to secure your reservation. The website will guide you through the reservation process. Your credit card will be used only to secure the on-line reservation and will not be charged unless you indicate this is your method of payment when you check-in at the Executive West Hotel. The Group ID # listed is valid for on-line reservations ONLY. All accessible rooms within the Hotel have been reserved and will be available on a ‘first come first served’ basis. Please be aware that this Hotel has taller than average mattresses and box springs in their rooms. The Hotel has agreed to remove the box springs for anyone requesting this service at the time of their reservation in an effort to make the room more accessible to the athletes and guests. If you anticipate the need for this accommodation please make this request at the time of your reservation so this can be taken care of before you arrive. Remember the $85.00 rate will not be available after April 1st so it is important to reserve your rooms early. FLIGHTS: All participants should fly into Louisville International Airport (SDF). Please check with the USQRA for any applicable airline or flight discounts available. If you are planning to attend the AGM Meeting scheduled for Thursday, April 10th please make arrangements to arrive on Wednesday, April 9th to ensure transportation will be available to you. Departing flights on Sunday, April 13th should be scheduled after 6:00PM. Transportation to the airport will also be provided on Monday, April 14th by availability and special arrangement ONLY and must be scheduled to leave by 12:00pm (est.). If your flight is scheduled after 12:00pm (est.) please be aware that transportation will only be available up until the 12:00pm (est.) time mentioned above. AIRPORT TRANSPORTATION: Accessible transportation will be provided to and from Louisville International Airport (SDF) for all participating teams, their support staff, AGM attendees and ‘Rugby Fans’. PLAYING VENUE TRANSPORTATION: Daily shuttle transportation will be provided to the playing venue beginning at 7:00am, Friday, April 11th through the Championship games on Sunday, April 13th for all competing teams, their support staff and ‘Rugby Fans’. All teams, their support staff, ‘Rugby Fans’ and AGM attendees must complete a “Transportation Request Form” (available on the USQRA website) the submission of this form is due by Friday, April 4th and can be faxed to (502) 582-7477 or e-mailed to jill.farmer@jhsmh.org. AGM: The USQRA Annual General Meeting (AGM) will take place on Thursday, April 10th at 9:00am at the Executive West Hotel. Each team affiliated with the USQRA is encouraged to send a representative to participate and vote on the important issues that pertain to the Sport and the Association. You must complete an AGM Registration Form (available on the USQRA website) and return it by Wednesday, April 9th in order to attend this meeting. PLEASE NOTE: If your team is NOT playing in the National Tournament you will only receive airport transportation to and from the hotel and lunch at the AGM unless you have secured one of the Rugby Fan Tournament Package options. COACHES MEETING: There will be a Coaches meeting immediately following the AGM on Thursday, April 10th at Executive West Hotel. Coaches who cannot attend this meeting should have a Team Representative present in order to receive important information pertaining to the tournament itself. ATTENDING TEAMS: We would like to congratulate you for qualifying to compete in the USQRA National Championship Tournament held in Louisville Kentucky. Please complete and return a Team Registration and Roster Form (available on the USQRA website). Deadline for the submission of this form is Friday, April 4th and can be faxed to (502) 582-7477 or e-mailed to jill.farmer@jhsmh.org or karey.mcdowell@jhsmh.org. RUGBY FAN TOURNAMENT PACKAGES: Everyone wants a part of the action at the 2008 Tournament. You can be an official “Rugby Fan’ by purchasing one of the available Packages ranging in price from $50.00 to $100.00 based on your individual or family/friend’s interests. To sign up as a ‘Rugby Fan’ please complete the registration form available on the USQRA website. Rugby Fan Tournament Packages can be purchased now through the start of the National Tournament. To register early please fax your completed form to (502) 582-7477 or mail it with your payment to Frazier Rehab Institute, Recreation Therapy Department-11th Floor, 220 Abraham Flexner Way, Louisville, Kentucky, 40202. Please make all checks payable to Frazier Rehab Institute-Adapted Sports Fund PLAYING VENUE: All games for the 2008 USQRA National Championship Tournament will be held in the Broadbent Arena at the Kentucky Fair and Exposition Center. The venue is located approximately 1-mile from the Host Hotel and a regular transportation shuttle service will be provided to all competing teams and their support staff to and from the games throughout the weekend. We encourage all other family members and spectators to purchase a Rugby Fan Tournament Package in order to make use of the transportation shuttles to and from the playing venue since there is a parking charge each time you enter the fairgrounds otherwise. We encourage everyone to make use of this opportunity in an effort to support the event itself. A Rugby Fan Tournament Package Form can be found on the USQRA website.
