USQRA Athlete of the Year and
                  Spirit of Achievement Awards
                                             Online Nomination Ballot

Each season the USQRA presents an award to the USQRA Athlete of the Year, and to a volunteer in recognition of their unselfish contribution to the sport of Quad Rugby. Recipients of both awards are selected by tallying up the nominations that are submitted to the USQRA President. Winners will be announced at the USQRA National Championship Awards Banquet.

You may use this form to submit your nomination for either, or, both awards electronically. Anyone associated with the USQRA in any way is encouraged to participate. You must abide by the following rules for your ballot to be considered legal.

  1. You may only nominate one person for each award.
  2. You must provide your real name and a phone number for your ballot to count
  3. You must state your reasons (see below) for nominating the person(s).
  4. Nominations must be submitted no later than March 10th.

Please tell us the reasons for your nomination as well as the nominees accomplishments. Take into account the following: Sportsmanship, All Tournament team selections, MVP selections, National team participation, contributions to the community and to the sport of quad rugby.

Spirit of Achievement Award
You may nominate anyone who is in the sport as a support person, coach, classifier etc., as long as they are a volunteer and not in a paid position. Please list your reasons for your nomination, as well as the nominees accomplishments.
